Based on the daily activities and needs for our residents, recommended visiting hours are 8:00 a.m. until 8:00 p.m. This provides our residents with the time they need to prepare for their day and obtain the rest they need to participate in their treatment and daily activities. Special accommodations can be arranged given special circumstance.
Our Admissions team accepts admissions seven days a week.

Email AdmissionsNestled away in a quiet countryside setting, the newly developed BellTower Health and Rehabilitation Center offers short-term rehabilitation, respite and long-term care. We know that any change can be difficult; especially the changes caused by illness or injury. That is why our trained nursing and therapy staff will work collaboratively with you and your independent physician to work towards your rehabilitation goals.
BellTower Health and Rehabilitation Center is a 100-bed premier rehabilitation and senior care community, licensed by the state of Indiana, providing long-term and short-term care. We believe that wellness is the result of comprehensive care and attention to service. We are committed to providing quality and supportive care to our residents and families.
Our premier rehabilitation suites offer a calm, therapeutic place to recover following surgery or illness and our clinical team will evaluate each resident's unique circumstances to develop an individual treatment plan. During your stay our therapy staff will work toward improving independence, strength, and confidence.
BellTower Health and Rehabilitation Center is focused on rehabilitation opportunities. We offer a wide range of rehabilitation services that are tailored to each resident's needs. The Rehabilitation Department offers services by independent licensed professionals in physical therapy, occupational therapy and speech pathology. These independent therapists create individualized therapy plans to promote wellness and recovery. The therapy department's goal is to facilitate independence for a successful return to the resident's prior level of functioning.

BellTower Health and Rehabilitation Center has the most up-to-date therapy equipment, and offers extensive independent therapies to meet virtually any rehabilitation need.
We offer services required to help those individuals in short-term or long-term complex rehabilitation. Our independent, licensed therapists and assistants provide comprehensive rehabilitative treatment in three primary disciplines which include physical, occupational, and speech therapy.
BellTower Health and Rehabilitation Center is devoted to discovering each individual's full functional capabilities as well as aiding and achieving each resident's highest practical level of independence possible.
Individualized therapy and treatment is provided by independent licensed clinicians in our spacious and fully equipped therapy gym. Our facility is designed with the very latest therapy assessment, monitoring, and treatment modalities. Rehabilitation strategy is tailored to the individual. We do this by simulating an environment that resembles home as much as possible in our ADL kitchen, bathroom, laundry areas, transition suite, and mobility garden.
Through independent licensed therapists and assistants, BellTower Health and Rehabilitation Center provides a full range of services including:
The resident's interdisciplinary team supports and designs care plans that seek to heal our resident's wounds under the direction and guidance of an independent physician. Our licensed wound care nurses work seven days a week for residents requiring treatment for complex wounds, including but not limited to post-operative wound sites, diabetic, and decubitus wounds.
Long-Term Care is for those who could benefit from 24-hour nursing, meal assistance, medication management, or other activities of daily living. Our facility is designed to provide our residents with a comfortable living environment and allows them to be a part of our community while we manage their health and wellness.
At BellTower Health and Rehabilitation Center, 24-hour skilled nursing care is provided by a team of nurses and nursing assistants. Our nursing staff includes Registered Nurses, Licensed Practical Nurses, and Certified Nursing Assistants. Our wound care nurses and restorative aides can help residents meet their highest level of functioning. Our case management team shares the same passion to help residents return to their community and/or achieve their highest level of functioning.
BellTower Health and Rehabilitation Center is very proud of the various recreational activities it offers for its residents. Each resident is individually assessed to provide a program of activities that addresses his or her physical, spiritual, emotional, recreational and social needs. Our certified Activity Director and volunteers encourage many levels of independence.
Music, arts, crafts, games, religious services and outings are all part of our rehabilitation program constructed with our residents in mind. A monthly calendar of events, as well as reminders, is given to our residents so they are well informed about everything BellTower Health and Rehabilitation Center has to offer.
Our activity programs often include field trips into the community. Trips can include places such as restaurants, movies, malls, and community events. The activities department can also help provide personal shopping assistance and the facility provides transportation to and from the various destinations.
BellTower Health and Rehabilitation Center's Dining menu follows a liberalized philosophy supported by the Academy of Nutrition and Dietetics. Our Culinary Services Director will meet with you to discuss any specific food restrictions and/or personal preferences.
We provide breakfast, lunch, dinner, and snacks to all of our residents. We encourage socialization among our residents and offer the option of dining in our beautiful and spacious dining areas. However, if residents prefer to eat in the comfort of their room, they are more than welcome to do so.
We pride ourselves in our ability to provide a person-centered approach to our residents' dining experience. Residents are served restaurant style and their individual selection is provided by our staff at meal time. All meals are prepared fresh daily and specifically created to suit all residents' individual needs, preferences, or modified texture diets.
